THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Midwest Cardiovascular Ultrasound is required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of your protected health information (PHI), provide this Notice, and abide by its terms.
Uses and Disclosures of Your PHI
We may use and disclose your PHI for:
- Treatment: To provide diagnostic services (e.g., echocardiograms, carotid Doppler) and coordinate care, including sharing with referring physicians.
- Payment: To bill and collect payment from you, your insurance, or third parties.
- Health Care Operations: For quality assessment, staff training, compliance, and business management.
Other permitted uses/disclosures without authorization:
- As required by law (e.g., public health reporting, abuse/neglect).
- For health oversight, judicial proceedings, law enforcement, coroners, or organ donation.
- To avert serious threats to health/safety.
- For workers’ compensation or specialized government functions.
Uses requiring opportunity to agree/object:
- To family/friends involved in your care or payment.
- For disaster relief or notifications.
All other uses/disclosures require your written authorization.
Your Rights Regarding Your PHI
- Inspect and obtain a copy (fees may apply).
- Request amendments.
- Receive an accounting of certain disclosures.
- Request restrictions (we may deny).
- Request confidential communications.
- Obtain a paper copy of this Notice.
- Be notified of any breach of unsecured PHI.
Complaints
If you believe your privacy rights were violated, complain to us or the U.S. Department of Health and Human Services (no retaliation).
